Welcome to Peak Performance Hydration. By booking or receiving any of our services, you agree to the following terms and conditions. Please read them carefully before your appointment.

1. Services Provided

Peak Performance Hydration provides IV hydration therapy, vitamin infusions, and related wellness services. All treatments are administered by qualified medical professionals and are intended for wellness support, not as a substitute for professional medical diagnosis or treatment.

2. Medical Intake & Consent

• All clients are required to complete a medical intake form and provide accurate, up-to-date health information prior to receiving services.

• Treatments will be customized based on your intake form, lifestyle, and any specific medical needs discussed during your consultation.

• By receiving services, you confirm that you have disclosed all relevant medical history, allergies, and medications.

3. Appointment Booking & Cancellations

• Appointments must be booked at least 1 hour in advance to allow infusion specialists time to prepare.

• We kindly request a minimum of 24 hours’ notice for cancellations or rescheduling.

• Late arrivals may result in shortened treatment time or rescheduling, at our discretion.

4. Payments & Refunds

• Payment is due at the time of service.

• Due to the nature of our treatments, all sales are final.

• Pain, bruising, mild discomfort, or lack of perceived benefit do not constitute grounds for a refund.

5. Risks & Acknowledgement

• IV hydration and vitamin infusions carry potential risks, including but not limited to bruising, swelling, pain at the injection site, allergic reactions, and infection.

• While uncommon, adverse reactions may occur. If you experience severe side effects, seek medical attention immediately.

• By receiving services, you acknowledge these risks and agree to hold Peak Performance Hydration and its staff harmless from any liability related to your treatment.

6. Privacy & HIPAA Compliance

• We are committed to protecting your personal and medical information in accordance with HIPAA guidelines.

• Your information will only be shared with our licensed medical direction — including telehealth consultations with our medical provider — for the purpose of evaluating, approving, or modifying your treatment plan.

• Your information will not be shared with any other third party without your written consent, except as required by law.

7. Membership Terms

• Membership benefits, discounts, and promotions are valid only for active members in good standing.

• Memberships are non-transferable and non-refundable.

• Peak Performance Hydration reserves the right to modify membership benefits with prior notice.

8. Disclaimer

• Our services are intended for wellness support and should not replace medical care from your primary healthcare provider.

• Always consult with your physician before beginning any new wellness treatment, especially if you have underlying medical conditions or are pregnant/nursing.

9. Changes to Terms

Peak Performance Hydration reserves the right to update or modify these Terms & Conditions at any time without prior notice. Updated terms will be posted in our clinic and on our website.
